The Homeless Health Care program is a multi-disciplinary approach to delivering care to homeless persons and/or families with children, combining outreach with the provision of primary care, mental health, substance abuse counseling, client advocacy and referrals for oral care. These services are available, at no charge, at locations accessible to homeless people. Particular emphasis is placed on coordinating efforts with local agencies.
Eligibility Criteria
- Currently homeless as defined by the McKinney Act
- Have used the services of a homeless shelter and / or soup kitchen in the last six months
- At risk for being homeless (any” yes" response to the following questions)
- Has recently been released from an institutional residential facility and no permanent place to sleep overnight?
- An adult sharing a residence with family/non-family other that his/her nuclear family in an overcrowded house?
- Multiple residences (more than two) in the last six months?
