Sliding Fee Scale
Optimus Health Care, Inc. offers a sliding fee scale for patients without health insurance coverage. Under this program, we calculate cost to be charge for services according to family income. Family income at and below the federal poverty level is charged minimum cost. For income guidelines, please call or visit any of our service sites nearest you. We require proof of income at time of visit to avail of this program.

Outreach
To ensure that we provide services to all those living within our service areas, Optimus Health Care, Inc. conducts outreach activities in our neighborhoods such as community forums, help desk, participation in community health fairs, public and mass media campaigns, among other things. Presumptive Eligibility and Application Assistance to Public Health Coverage
No health insurance coverage? Not a problem. Optimus Health Care, Inc can help you determine if you are eligible for any public health assistance coverage such as Medicare, Medicaid, HUSKY or other local health benefits. We have in-house social workers and outreach workers ready to help you navigate through the screening and application process for these benefits. A Department of Social Services staff is available at some of our locations to process application and gives on-site approval. Under presumptive eligibility, you do not have to delay your health care while waiting for decision on your public health coverage application. Optimus Health Care, Inc. works with Sacred Heart University and the St. Charles Roman Catholic Church on East Main Street in Bridgeport to provide free health education, blood pressure and blood sugar monitoring, medication management and other health promotion activities to the neighborhood. Services are provided by faculty and students of the Sacred Heart University and our outreach coordinator is available to provide direct linkage to our health center services as needed. Foster Care Multi-Disciplinary Evaluation
Optimus Health Care, Inc. conducts multi-disciplinary evaluation for children going into the foster care system for the first-time, as a service for the State of Connecticut Department of Children and Families. The multi-disciplinary evaluation covers medical, dental and behavioral health evaluations conducted within the same visit and facilitated by a coordinator. Foster Care Screening clinics are conducted at our East Main Street site in Bridgeport and at our Henry Street location in Stamford. Case Management/Social Workers
